Orthopaedic Surgeon, ENT Surgeon, Ophthalmic Surgeon,
Psychiatrist અથવા Child Disease Specialistમાંથી જરૂરિયાત મુજબ સંબંધિત specialist.

🏥 Specialist Availability:
ઠરાવમાં Board માટે જરૂરી specialist ઉપલબ્ધ ન હોય તેવી સ્થિતિમાં અન્ય Government
Hospital/Medical Collegeમાંથી specialistની સેવા મેળવવા તથા જરૂરી હોય તો નજીકની
Government institutionમાંથી વ્યવસ્થા કરવા માટે provision દર્શાવવામાં આવ્યું છે.

♿ Disability Certificate
અરજદાર પાસે પહેલેથી disability certificate હોય તો તેની copy સાથે અરજી.
Special Situation:
દિવ્યાંગ વ્યક્તિ ગંભીર/બહુવિધ અથવા અન્ય એવી disabilityને કારણે પોતે અરજી કરી
શકતી ન હોય અથવા જાતે જઈ શકતી ન હોય તો તેના કાયદેસર વાલી દ્વારા અરજી કરી શકાય છે.

2. District Assessment Board
ChairpersonChief District Medical Officer-cum-Civil Surgeon or the specified medical authority of a Medical College attached hospital.
Social SecurityDistrict Social Security Officer as a member.
Medical SpecialistRelevant specialist such as Orthopaedic, ENT, Ophthalmic, Psychiatric or Child Disease specialist according to the disability concerned.
Rehabilitation ProfessionalOccupational Therapist, Physiotherapist, Clinical Psychologist or other relevant professional as required.
