આ ઠરાવ હેઠળ NPSમાં સમાવિષ્ટ પાત્ર રાજ્ય સરકારના અધિકારી/કર્મચારીઓને અશક્તતાને કારણે નિવૃત્તિના કિસ્સામાં Gujarat Civil Services (Pension) Rules, 2002 હેઠળ disability/invalidation pension સંબંધિત લાભ અને ચાલુ નોકરી દરમિયાન અવસાનના કિસ્સામાં family pension સંબંધિત લાભ માટે optionની વ્યવસ્થા કરવામાં આવી છે. નિર્ણયને 01-04-2005ની અસર આપવામાં આવી છે.

Option 1 — Pension Rules Benefits
અશક્તતાને કારણે retirement અથવા ચાલુ નોકરીએ death થાય ત્યારે કર્મચારી/પરિવારને Gujarat Civil Services (Pension) Rules, 2002 મુજબ લાભ મળે તેવી પસંદગી.Option 2 — NPS Corpus Benefits
Individual Pension Accountમાં accumulated NPS pension corpusના આધારે, રાજ્ય સરકાર દ્વારા સમયાંતરે notify થતા rules મુજબ લાભ મેળવવાની પસંદગી.Form-A અને 3 મહિનાની સમયમર્યાદા
Form-13 — પરિવારની વિગતો
Option સાથે GCS Pension Rules, 2002ના Rule 89 હેઠળ prescribed Form-13માં family details આપવાની છે. PDFના Page 6 પર Form-13નું specimen છે, જેમાં family memberનું નામ, જન્મ તારીખ, Government employee સાથેનો સંબંધ, Head of Officeની સહી અને remarks માટે columns છે.

This resolution provides an option for eligible State Government officers/employees covered by NPS to receive benefits under the Gujarat Civil Services (Pension) Rules, 2002 in cases of retirement on invalidation and death during service, including applicable family-pension benefits. The decision is given effect from 01-04-2005.

Choose benefits based on the accumulated pension corpus in the Individual Pension Account under NPS, in accordance with rules notified by the State Government.Form-A and Three-Month Time Limit
Form-13 — Details of Family
Along with the option, family details are to be furnished in prescribed Form-13 under Rule 89 of the GCS Pension Rules, 2002. Page 6 of the PDF contains the Form-13 specimen with columns for family member name, date of birth, relationship with the Government employee, Head of Office signature and remarks.
